Community Health Nurse

Reports to a Director/Manager or Supervisor.
Provides health services by implementing/performing care/services as needed for designated community initiatives.
Also serves as a resource to participants at all levels to support such initiatives of the Community Health Service Line.

Promotes and supports assigned Community Health initiative by:
Performing health assessments and administering mandated screenings.
Screen and provide education to participants of initiative.
Consulting with individual health appraisals and the appropriate follow up care.
Providing appropriate care to participants by administering first aid, medications, vaccinations and/or specialized medical treatments according to physician's instructions.
Counseling participants as needed based on initiative.
Performs all necessary recording keeping required to track Community Health initiative.
Maintains knowledge, skills and competence in areas of responsibility by:
Attending in-services and department meetings; also participating in continuing education and attending conference as approved.
Attend professional meetings as assigned.
Successfully completes skills validation as required.
Contributes to the overall effectiveness and efficiency of the department by:
Acting as a liaison to community agencies and medical care facilities.
Implementing and maintaining all health records (if applicable).
Cleans work areas (utilizing universal precaution procedures) for the purpose of maintaining a sanitary environment.
Completing other job-related duties and projects as assigned.
ORGANIZATIONAL RESPONSIBILITIESAssociate complies with the following organizational requirements:
Attends and participates in department meetings and is accountable for all information shared.
Completes mandatory education, annual competencies and department specific education within established timeframes.
Completes annual employee health requirements within established timeframes.
Maintains license/certification, registration in good standing throughout fiscal year.
Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
Adheres to regulatory agency requirements, survey process and compliance.
Complies with established organization and department policies.
Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and ExperienceThe above level of knowledge, skills and abilities are normally acquired through the successful graduation from an accredited school of nursing and have a current Registered Nurse license in the state of Indiana.
Previous experience in school nursing, public health, community nursing or pediatrics is desired.
Must have and maintain current CPR certification.
A valid driver's license and a good driving record are required.
Knowledge & Skills Demonstrates a high level of knowledge and competency in performing nursing care procedures, assessments, treatment and techniques.
Demonstrates analytical skills necessary to solve problems and interpret data.
Requires the ability to work well with others; organizational ability to establish and maintain necessary systems and records; ability to work independently.
Demonstrates the interpersonal and communication skills (both verbal and written) necessary to interact effectively with a wide variety of contacts including principals, students, families, and community members; also to report and record patient related information accurately and appropriately.
Requires strong documentation and organization skills to multi-task and maintain electronic health records of students.
Demonstrates evidence of physical health and physical exam upon hire and as needed.
Must complete annual TB test.
Demonstrates proficiency in basic computer skills (i.
e.
, data entry, word processing and spreadsheets).
Must have working phone and/or pager to maintain contact with agency.
Requires a valid driver's license, access to a vehicle, and proof of automobile liability insurance coverage.

Working Conditions Works in an office or school environment.
Travel to different locations in the surrounding community.
Physical DemandsRequires the physical ability and stamina (i.
e.
to walk/stand for prolonged periods of time, push carts/wheelchairs up to 50 pounds, to position/lift patients at a maximum of 35 pounds unassisted, over 35 pounds requires assistance, provide CPR, etc.
) to perform the essential functions of the position.
